What Happens on Surgery Day — A Full Arch Case From Start to Finish

Full arch implant surgery is our most transformative procedure. Here is what the day looks like. You arrive early morning. IV sedation is started. While you are deeply relaxed, Dr. Ostovar extracts any remaining failing teeth, debrides the sockets, and smooths the alveolar ridge to create a flat, even surface for the prosthesis. Then, using a CBCT-planned surgical guide, he places four to six implants at precise positions and angles. Insertion torque is checked — each implant needs at least 35 Ncm for same-day loading.

Once all implants are placed and primary stability is confirmed, multi-unit abutments are screwed onto each implant to correct the angulation between the implant and the prosthesis. Impressions are taken or a pre-fabricated provisional bridge is tried in, adjusted, and secured to the abutments with small prosthetic screws. The access holes are sealed with temporary filling material. You wake up from sedation with a full set of fixed teeth that do not come out.

The provisional bridge is made from reinforced acrylic — durable enough for soft foods during the three-to-six-month osseointegration period. During this time, you return for periodic checkups to ensure healing is on track. Once the implants are fully integrated, the provisional is removed, final impressions are taken, and a permanent bridge — typically milled monolithic zirconia with individually characterized teeth — is fabricated and delivered. This final prosthesis is designed for 15 to 25 years of hard use.

The provisional is a reinforced acrylic bridge designed for the four-to-six-month healing period. It is functional and aesthetic but softer than the final restoration. The final prosthesis is typically milled monolithic zirconia — extremely hard, stain-resistant, and durable for 15 to 25 years. It features individually characterized teeth and natural-looking gum contours.

How many teeth are on a full arch prosthesis?

Typically 10 to 14 teeth per arch, depending on your facial anatomy, smile line, and the position of the implants. Most patients receive 12 teeth per arch. The prosthesis also includes gum-colored material that simulates natural gingival contours, especially important when bone loss has altered the ridge height.

Can the final prosthesis be removed for cleaning?

The patient cannot remove it — it is secured with prosthetic screws that only a dentist can access. At your regular maintenance visits (every six months), we can remove the prosthesis, clean it thoroughly along with the implants and abutments underneath, and reseat it. Between visits, you clean around it with a brush, water flosser, and interdental aids.

Will I need someone to care for me after surgery?

How often do I need follow-up appointments after full arch surgery?

Expect checkups at one week, two weeks, one month, three months, and six months post-surgery. After the final prosthesis is delivered, routine maintenance visits every six months are recommended. What is the difference between this procedure and implant-supported dentures?

Full arch implants produce a permanently fixed prosthesis that is screwed onto the implants and does not come out. Implant-supported dentures snap on and off — the patient removes them nightly for cleaning. Full arch implants are stronger, more natural-feeling, and better at preserving bone, but they cost more.

Can I get full arch implants if I have gum disease?

Yes, but any active periodontal disease must be treated before or during the procedure. In full-arch cases, all remaining teeth (and the infection they harbor) are removed during surgery. The surgical field is thoroughly debrided, and the implants are placed in clean bone. Post-operatively, the fixed prosthesis is easier to keep clean than diseased natural teeth were.

What if I do not like the way my new teeth look?

The provisional prosthesis is your trial period. If you want adjustments to tooth shape, size, color, or arrangement, we make those changes before fabricating the final prosthesis.
