What to Bring
- Your current treatment plan (printed or digital)
- Any X-rays or CT scans from other offices (we can also take new 3D imaging at no cost)
- List of questions or concerns about the proposed treatment
- Insurance card (we will verify benefits as part of the consultation)
If you do not have your records, we can request them from the other office on your behalf with your written consent.
