Ridge Augmentation (Vertical) - Dental Implants at Aloha Dental Specialty Center in Beaverton & Hillsboro, OR

Ridge Augmentation Vertical

Ridge Augmentation Vertical is a specialized dental service provided by the board-certified specialists at Aloha Dental Specialty Center in Beaverton, OR. Walk in with failing teeth. Walk out with new ones. Same-day implant loading delivers functional teeth immediately.

Same-Day Teeth

Teeth-in-a-Day Is Not Marketing Hype — It Is an Evidence-Based Protocol

Immediate load implants — often called "teeth in a day" — refers to the practice of attaching a functional prosthesis to dental implants on the same day they are surgically placed. Instead of placing implants, closing the tissue, and waiting three to six months before receiving teeth, the implants are loaded with a temporary restoration within hours of surgery. You leave the office with teeth that are fixed in place and functional for a soft diet.

The protocol requires specific conditions to be safe: each implant must achieve primary stability of at least 35 Ncm insertion torque, the prosthesis must be designed for passive fit to avoid inducing strain on healing implants, and the patient must commit to a soft-food diet during the integration period. When these criteria are met — which they are in the majority of full-arch cases and many single or multi-tooth cases — immediate loading is not just safe, it is the standard of care at our practice.

The psychological benefit is immediate. Patients who have suffered with failing teeth, ill-fitting dentures, or missing teeth for months or years walk out of our office the same day with a complete set of stable, aesthetic teeth. They can smile, speak, and eat soft foods that evening. There is no extended period of being toothless. Dr. Ostovar has performed hundreds of immediate-load cases and considers it one of the most rewarding procedures in his practice because the transformation is visible before the patient leaves the chair.

Understanding Your Investment

At Aloha Dental Specialty Center, we provide transparent pricing before any treatment begins. The cost of your procedure depends on clinical complexity, materials used, and sedation requirements. We walk you through every line item during your consultation so there are no surprises.

What Affects Cost:

  • Clinical Complexity: Bone loss, sinus proximity, nerve involvement, or the need for grafting affects treatment planning and surgical time.
  • Materials: We use research-backed implant systems, purified bone graft matrices, and high-strength ceramics from established manufacturers.
  • Sedation: Options range from local anesthesia to IV sedation, each with different associated costs. We discuss the best option for your comfort and procedure.

Insurance & Financing

We accept most major dental insurance plans and file claims on your behalf. Our team verifies your benefits before treatment and submits pre-treatment estimates with clinical documentation to maximize coverage.

For out-of-pocket costs, we partner with CareCredit and Cherry for flexible payment plans. We also offer our ADSC Dental Savings Plan for patients without insurance.

Plan for approximately 1-2 hours for your ridge augmentation vertical visit, including preparation and post-procedure instructions. Recovery varies by individual — we’ll give you a specific timeline based on your case.

Recovery & Healing Timeline

Understanding the healing process helps you plan ahead and follow post-operative instructions for the best possible outcome.

First 48 Hours

Mild swelling and tenderness are normal. Apply ice packs in 20-minute intervals and take prescribed or over-the-counter anti-inflammatory medication. Eat soft foods and avoid the surgical site when chewing.

Days 3 to 7

Swelling peaks around day three and then improves. The tissue begins closing over the surgical site. Continue with soft foods and use any prescribed antimicrobial rinse. Most patients return to normal activities during this phase.

Long-Term Healing

Surface tissue heals within two to three weeks. Bone integration and deep healing continue for three to six months. Avoid smoking, follow up as scheduled, and maintain good oral hygiene throughout recovery.

Questions during recovery? Call us at (503) 822-0096. We are available for post-operative concerns.

The Science

Primary Stability and Micromotion: The Science Behind Same-Day Loading

Successful osseointegration requires limiting micromotion at the bone-implant interface to below 50 to 150 micrometers during the healing period. If an implant moves excessively before integration, fibrous tissue forms instead of bone, and the implant fails. Primary stability — the mechanical hold achieved at insertion — is the key determinant of whether immediate loading is safe. Insertion torque above 35 Ncm and implant stability quotient (ISQ) values above 65 correlate with successful immediate loading. Splinting multiple implants with a rigid framework further reduces micromotion at each individual implant by distributing occlusal forces as a unit. Research from Malo, Lazzara, and others demonstrates that immediate-load protocols achieve implant survival rates of 95 to 99 percent — statistically equivalent to conventional delayed-loading protocols.

Experience & Expertise

Why Choose a Specialist?

The decision to load an implant immediately is a clinical judgment that weighs primary stability, bone quality, occlusal forces, and the specific prosthetic design. It is not a decision that should be made by default or marketing pressure. Dr. Ostovar measures insertion torque and assesses stability for every implant before committing to immediate loading. If any implant does not meet the threshold, he converts it to a submerged healing protocol — prioritizing your long-term outcome over the appeal of same-day results. This willingness to adjust the plan intraoperatively is the hallmark of experience.

Treatment time varies by complexity. A straightforward procedure may take 30-60 minutes, while more involved cases can take 1-2 hours. Your consultation visit typically lasts 45-60 minutes including imaging and treatment planning. We’ll give you a specific time estimate for your individual case.

A Note from Your Dental Team

In my experience, the patients who get the best results from ridge augmentation vertical are those who come in with realistic expectations and follow their post-treatment instructions carefully. I’d rather spend extra time explaining what to expect than have a patient be surprised later.

“Every patient I see has a different story, a different set of concerns, and a different definition of what ‘success’ looks like. That’s why I don’t believe in one-size-fits-all treatment plans. When you come in for a consultation, I’ll listen first, examine second, and recommend third.

I also won’t recommend a procedure you don’t need. If your tooth can be saved with a filling instead of a crown, I’ll tell you that. If watchful waiting is appropriate, I’ll explain why. My job isn’t to sell treatment — it’s to give you my honest clinical judgment so you can make an informed decision.

If you have questions about whether this treatment is right for you, or if you’ve been told you need this procedure by another dentist and want a second opinion, call us at (503) 822-0096. We’re happy to take the time to explain everything — no rush, no pressure.”

— Dr. Merat Ostovar & Dr. Jovan Gvozden | Aloha Dental Specialty Center, Beaverton, OR

I know many patients feel anxious about dental procedures — you’re not alone in that. What I hear most often after treatment is: “That was so much easier than I expected.” We’re here to make this as comfortable as possible for you.

Typical Healing Timeline:

Days 1-3: Initial healing, mild swelling managed with ice and medication. Days 4-7: Swelling subsides, gradual return to normal diet. Weeks 2-4: Soft tissue heals completely. Months 2-6: Bone remodeling and full integration (for surgical procedures). Individual recovery varies — we provide detailed post-op instructions specific to your procedure.

How long does it take? Most patients complete this procedure in approximately 45-90 minutes. Your initial consultation is approximately 60 minutes including CBCT 3D imaging and treatment planning.

Frequently Asked Questions

What does "teeth in a day" actually mean?

It means dental implants are placed and a functional prosthesis (temporary teeth) is attached to them in a single appointment. You arrive with failing or missing teeth and leave with a new set of fixed teeth. The temporary prosthesis is replaced with a permanent one after the implants fully integrate, typically in four to six months.

Is same-day loading safe for the implants?

Yes, when strict criteria are met. Each implant must achieve a minimum insertion torque of 35 Ncm, and the prosthesis must be designed for passive fit. Under these conditions, published survival rates for immediately loaded implants are equivalent to conventionally loaded implants — both above 95 percent.

Can any implant be immediately loaded?

Not all. Immediate loading requires sufficient primary stability, which depends on bone density and quality. Implants placed in very soft bone, grafted sites, or severely atrophic ridges may not achieve the stability threshold. Dr. Ostovar assesses each implant individually and only loads those that meet the criteria.

What kind of teeth do I get on the same day?

A reinforced acrylic provisional prosthesis — durable enough for soft foods and fully aesthetic. It is tooth-colored, shaped to your specifications, and attached firmly to the implants. This provisional is worn for four to six months during healing and is then replaced with the final restoration (typically monolithic zirconia).

Can single implants be immediately loaded?

Yes, in select cases. Single implants in the aesthetic zone (front teeth) are often immediately loaded with a provisional crown to maintain the soft-tissue contour during healing. The provisional crown is adjusted out of occlusion so it does not bear heavy bite forces. Posterior single implants are less commonly immediately loaded.

What diet restrictions apply after same-day teeth?

Soft foods only for the first three months: scrambled eggs, fish, pasta, yogurt, cooked vegetables, smoothies, and soups. No biting into hard foods (apples, crusty bread) or chewing tough items (steak, raw carrots). The soft diet protects the implants during the critical osseointegration period. Full diet resumes after the final prosthesis is placed.

How is same-day loading different from "same-day implants"?

These terms are often used interchangeably. "Same-day implants" usually refers to immediate implant placement (placing an implant the same day a tooth is extracted). "Same-day loading" or "teeth in a day" refers to immediately attaching a prosthesis to the implants. They are different concepts, though both can occur in the same appointment.

What happens if an implant does not have enough stability for same-day loading?

Dr. Ostovar will not load an unstable implant. If one implant in a multi-implant case falls short, it is buried under the gum to heal on its own (submerged protocol), while the remaining implants support the provisional prosthesis. If a single implant does not qualify, conventional delayed loading is used instead.

Does immediate loading have a higher failure rate?

No. When proper selection criteria are applied, immediate loading has failure rates equivalent to delayed loading — both in the 1 to 3 percent range. The key is case selection and surgeon experience. Improper immediate loading (on unstable implants or without rigid splinting) does increase risk, which is why strict protocols exist.

How soon can I go back to work after teeth-in-a-day?

Most patients take two to five days off, depending on the extent of the procedure. Single-arch cases with IV sedation typically allow a return to work within two to three days. Both-arch reconstructions may require four to five days. The provisional teeth are in place immediately, so appearance is not a barrier to returning to work.

Can I get immediate-load implants if I currently have dentures?

Yes. Patients transitioning from dentures to fixed implant teeth are excellent candidates for immediate loading. Your existing denture can sometimes be converted into the provisional prosthesis, or a new provisional is fabricated to fit the implant positions. Either way, you leave with fixed teeth the same day.

How much does teeth-in-a-day cost?

Costs are similar to standard full-arch implant treatment — $20,000 to $30,000 per arch — because the implants and prosthesis are the same; only the timing of loading differs. There is no additional fee for immediate loading. The value is in the reduced number of appointments and the elimination of a toothless healing period.

What is the long-term success rate of immediate-load implants?

Ten-year data from multiple studies shows survival rates of 95 to 99 percent for immediately loaded implants in full-arch applications, and 93 to 97 percent for immediately loaded single implants. These rates are statistically equivalent to delayed-loading outcomes. Immediate loading is not a shortcut — it is an evidence-based protocol.

Do I still need to come back for a permanent prosthesis?

Yes. The same-day teeth are provisional — designed for the three-to-six-month healing period. Once the implants are fully integrated, the provisional is removed and replaced with a permanent prosthesis (typically milled zirconia). The permanent restoration is stronger, more aesthetic, and designed for decades of use.

What if I do not like the look of my same-day teeth?

The provisional prosthesis serves as a trial run for the final restoration. If you want changes to tooth size, shape, arrangement, or color, we adjust the provisional and use it as a template for the final prosthesis. This process ensures you are completely satisfied with your appearance before the permanent teeth are fabricated.
