Preventing bone collapse after tooth removal to keep your options open for implants.

Ridge Preservation

What You Do at the Moment of Extraction Determines What's Possible Later

The clock starts the moment a tooth leaves its socket. Within the first three months, the buccal plate — the thin outer wall of bone — begins to resorb. By twelve months, the ridge may have lost 40-60% of its width. This collapse is not theoretical; it is a well-documented biological certainty. Socket preservation grafting intervenes at the point of maximum opportunity: immediately after extraction, while the socket walls are intact, while the blood supply is robust, and before the body has begun dismantling the bone structure it no longer deems necessary.

At Aloha Dental Specialty Center, socket preservation is integrated into extraction planning from the outset. If an implant is planned — or even possible in the future — we graft the socket at the time of extraction as a matter of protocol. The technique involves gently extracting the tooth (preserving all socket walls), debriding the socket of granulation tissue, packing it with particulate bone graft material mixed with PRF, covering the opening with a collagen membrane and additional PRF, and suturing the tissue edges to contain the graft. The materials Dr. Ostovar selects depend on the socket anatomy and the timeline for implant placement.

The grafted socket heals over 4-6 months. During this time, the particulate graft acts as a scaffold — your osteoblasts migrate into the spaces between graft particles and lay down new bone. PRF accelerates this process by concentrating growth factors directly at the site. When healing is complete, a CBCT scan confirms the bone volume and density, and implant placement can proceed with confidence that the ridge has maintained its dimension. The alternative — extracting without grafting and finding a collapsed ridge months later — often means a more invasive and expensive ridge augmentation procedure before the implant can go in.

Understanding Your Investment

At Aloha Dental Specialty Center, we provide transparent pricing before any treatment begins. The cost of your procedure depends on clinical complexity, materials used, and sedation requirements. We walk you through every line item during your consultation so there are no surprises.

What Affects Cost:

  • Clinical Complexity: Bone loss, sinus proximity, nerve involvement, or the need for grafting affects treatment planning and surgical time.
  • Materials: We use research-backed implant systems, purified bone graft matrices, and high-strength ceramics from established manufacturers.
  • Sedation: Options range from local anesthesia to IV sedation, each with different associated costs. We discuss the best option for your comfort and procedure.

Insurance & Financing

We accept most major dental insurance plans and file claims on your behalf. Our team verifies your benefits before treatment and submits pre-treatment estimates with clinical documentation to maximize coverage.

For out-of-pocket costs, we partner with CareCredit and Cherry for flexible payment plans. We also offer our ADSC Dental Savings Plan for patients without insurance.

Recovery & Healing Timeline

Understanding the healing process helps you plan ahead and follow post-operative instructions for the best possible outcome.

First 48 Hours

Mild swelling and tenderness are normal. Apply ice packs in 20-minute intervals and take prescribed or over-the-counter anti-inflammatory medication. Eat soft foods and avoid the surgical site when chewing.

Days 3 to 7

Swelling peaks around day three and then improves. The tissue begins closing over the surgical site. Continue with soft foods and use any prescribed antimicrobial rinse. Most patients return to normal activities during this phase.

Long-Term Healing

Surface tissue heals within two to three weeks. Bone integration and deep healing continue for three to six months. Avoid smoking, follow up as scheduled, and maintain good oral hygiene throughout recovery.

Questions during recovery? Call us at (503) 822-0096. We are available for post-operative concerns.

The Science

Alveolar Ridge Resorption and the Role of Scaffolding

After extraction, the bundle bone — the layer of alveolar bone directly attached to the periodontal ligament — is the first to resorb because it depended on the ligament for its blood supply and stimulus. The buccal plate, typically only 0.5-1mm thick at anterior teeth, may be entirely bundle bone, meaning it can resorb completely within weeks. Socket preservation grafting works by maintaining the three-dimensional volume of the socket: the graft particles hold space mechanically, the membrane prevents soft tissue ingrowth into the socket (which would prevent bone formation), and the PRF provides biological signaling that recruits osteoprogenitor cells. The process is called guided bone regeneration in miniature — the same principles used in major augmentation procedures, applied at the moment when they require the least intervention.

Experience & Expertise

Why Choose a Specialist?

Socket preservation may appear straightforward — fill the hole with graft material — but the details determine outcomes. Was the tooth extracted atraumatically, or did the buccal plate fracture? Is the membrane positioned correctly to prevent graft particle loss? Is flap closure achieved without excess tension that could necrose the tissue and expose the graft? Was PRF prepared properly and placed to maximize growth factor release over the healing period? These subtleties require a surgeon who performs socket grafting as part of a daily surgical practice, not as an occasional add-on to a general dental visit.

A Note from Your Dental Team

In my experience, the patients who get the best results from socket graft ridge preservation are those who come in with realistic expectations and follow their post-treatment instructions carefully. I’d rather spend extra time explaining what to expect than have a patient be surprised later.

“Every patient I see has a different story, a different set of concerns, and a different definition of what ‘success’ looks like. That’s why I don’t believe in one-size-fits-all treatment plans. When you come in for a consultation, I’ll listen first, examine second, and recommend third.

I also won’t recommend a procedure you don’t need. If your tooth can be saved with a filling instead of a crown, I’ll tell you that. If watchful waiting is appropriate, I’ll explain why. My job isn’t to sell treatment — it’s to give you my honest clinical judgment so you can make an informed decision.

If you have questions about whether this treatment is right for you, or if you’ve been told you need this procedure by another dentist and want a second opinion, call us at (503) 822-0096. We’re happy to take the time to explain everything — no rush, no pressure.”

— Dr. Merat Ostovar & Dr. Jovan Gvozden | Aloha Dental Specialty Center, Beaverton, OR

I know many patients feel anxious about dental procedures — you’re not alone in that. What I hear most often after treatment is: “That was so much easier than I expected.” We’re here to make this as comfortable as possible for you.

Are You a Candidate for Socket Graft Ridge Preservation?

As with any dental procedure, there are potential risks including infection, temporary discomfort, and in rare cases, complications that require additional treatment. We minimize these risks through 3D imaging, sterile protocols, and careful case selection. During your consultation, we’ll discuss any specific risks related to your individual situation.

Most patients in good general health are candidates for socket graft ridge preservation. However, certain factors may affect your eligibility or require modifications to the treatment plan:

  • Medical conditions: Uncontrolled diabetes, autoimmune disorders, or blood-thinning medications may require coordination with your physician before treatment.
  • Bone quality: For socket graft ridge preservation involving the jawbone, adequate bone density is essential. A CBCT 3D scan during your consultation determines this precisely.
  • Smoking: Tobacco use significantly impairs healing. We strongly recommend quitting 2-4 weeks before and after any surgical procedure.
  • Age: There is generally no upper age limit. What matters is your overall health, not your age. We have successfully treated patients in their 80s and 90s.

The only way to know for certain is a consultation with our board-certified specialists. Call (503) 822-0096 to schedule your evaluation — we’ll tell you honestly whether this is the right treatment for your situation.

Typical Healing Timeline:

Days 1-3: Initial healing, mild swelling managed with ice and medication. Days 4-7: Swelling subsides, gradual return to normal diet. Weeks 2-4: Soft tissue heals completely. Months 2-6: Bone remodeling and full integration (for surgical procedures). Individual recovery varies — we provide detailed post-op instructions specific to your procedure.

Frequently Asked Questions

Why is socket preservation recommended after tooth extraction?

After tooth extraction, the surrounding bone resorbs rapidly — up to 50% width loss in the first 6 months. Socket preservation fills the extraction site with bone graft material to maintain bone volume for future implant placement. Without it, you may need more extensive (and expensive) ridge augmentation surgery later. We recommend socket grafting at every extraction site where an implant is planned.

Why is socket preservation important?

Without grafting, the extraction socket collapses as bone resorbs — losing up to 50% of its width within the first year. If you later want an implant, there may not be enough bone remaining, requiring a more invasive and costly ridge augmentation procedure. Socket preservation is the least invasive and most cost-effective way to maintain bone volume.

Does socket preservation add pain to the extraction?

Very little. The grafting is performed while you are still numb from the extraction anesthesia. The graft material is simply packed into the socket, a membrane is placed, and the tissue is sutured. Post-operative pain is comparable to extraction alone — the socket would be healing regardless; the graft does not significantly increase discomfort.

How long does a socket graft take to heal before implant placement?

Socket grafts typically heal in 3-4 months. During this time, the graft material integrates with your natural bone and new blood vessels form throughout the site. We verify healing with follow-up imaging before scheduling implant placement. The 3-4-month wait is well worth it — socket grafting provides a solid bone foundation for long-term implant success.

What material is used for socket preservation?

We typically use particulate allograft (processed human donor bone) or a combination of allograft and xenograft (bovine bone), mixed with PRF from your own blood. A resorbable collagen membrane covers the socket opening. The choice of material depends on the socket size, the condition of the socket walls, and the planned timeline for implant placement.

What material is used for socket preservation at ADSC?

We primarily use freeze-dried bone allograft (human donor bone processed and sterilized by FDA-regulated tissue banks) covered with a collagen membrane. In some cases, xenograft (bovine bone mineral) or synthetic bone substitute is used. The graft acts as a scaffold for your body to deposit new living bone. The material is completely safe with decades of clinical evidence.

Is socket preservation always necessary after extraction?

No. Socket preservation is most important when you plan to replace the tooth with an implant, when the extraction is in an aesthetically visible area, or when you want to preserve options for future restoration. If you are not replacing the tooth and it is in a non-visible area, grafting may be optional.

Is socket preservation painful?

Socket preservation is performed at the same time as your extraction, so you are already numb. Adding the graft material takes only 5-10 minutes. Post-operative discomfort is similar to a regular extraction — managed with over-the-counter ibuprofen for 2-3 days. The collagen membrane dissolves on its own and does not require removal.

Can socket preservation be done if the tooth was infected?

In many cases, yes. After the tooth is extracted, the socket is thoroughly debrided and irrigated to remove infected tissue. If the infection is mild and can be cleared surgically, grafting can proceed. In cases of severe active infection with extensive bone destruction, we may delay grafting until the infection resolves, then perform ridge augmentation as a second procedure.

What is PRF and why is it used with socket preservation?

PRF (platelet-rich fibrin) is a blood concentrate prepared from a small sample of your own blood. It is rich in growth factors that promote healing, new blood vessel formation, and bone cell recruitment. We mix PRF with the graft material and place PRF membranes over the site. This enhances the biological environment and accelerates graft integration.

Do I need to follow special instructions after socket preservation?

The post-operative instructions are similar to those for extraction: avoid straws, smoking, and vigorous rinsing for the first week. Do not poke at or disturb the graft site. Eat soft foods on the opposite side for several days. Gentle salt water rinses can begin after 24 hours. The membrane covering the graft may become visible as the tissue heals — this is normal.

What if the socket wall is broken during extraction?

If the buccal wall fractures during extraction, additional measures may be needed — such as using a collagen membrane on the outside of the socket to contain the graft material, or using a titanium-reinforced membrane for GBR. The graft can still be successful; it just requires adaptation of technique to the compromised socket anatomy.

Can socket preservation be done with a wisdom tooth extraction?

It is not typically done for wisdom tooth sockets because these sites are rarely used for implant placement and the bone in the retromolar area remodels adequately on its own. The exception is when the extraction has caused a significant bone defect on the distal surface of the second molar, in which case grafting may help preserve bone around that adjacent tooth.

Is socket preservation the same as bone grafting?

Socket preservation is a specific type of bone grafting. All socket preservations are bone grafts, but not all bone grafts are socket preservations. Socket preservation refers specifically to grafting at the time of extraction. Other bone grafts — ridge augmentation, sinus lift, GBR — address bone deficits that already exist from prior resorption.

How do I know the socket graft worked?

We verify graft success with a CBCT scan taken 4-6 months after the procedure. This three-dimensional image shows the bone volume, density, and dimensions at the grafted site. Clinically, the ridge should maintain its width and height without significant collapse. If the graft is successful — which is the case over 95% of the time — the site is ready for implant placement.
