Knocked-Out Tooth (Dental Avulsion) Treatment in Beaverton

A Knocked-Out Tooth Can Be Saved — But Only If You Act Fast

If a permanent tooth has been completely knocked out of its socket, the clock is ticking. Periodontal ligament cells on the root surface begin dying within 15 minutes of being outside the mouth, and every passing minute reduces the likelihood of successful reimplantation. Teeth reimplanted within 30 minutes have survival rates approaching 90 percent. After two hours, the survival rate drops below 5 percent. Pick up the tooth by the crown only — never touch the root — place it in cold milk or saline, and call (503) 822-0096 as you head to our office. We will have everything prepared for your arrival.

At Aloha Dental Specialty Center, Dr. Gvozden has reimplanted avulsed teeth successfully in hundreds of cases from sports injuries, falls, bicycle accidents, and assaults. The reimplantation protocol involves gently irrigating the socket, repositioning the tooth under direct visualization, and splinting it to the adjacent teeth with a flexible wire-composite splint for 7 to 14 days. We verify positioning with CBCT imaging and prescribe appropriate antibiotics and anti-inflammatory medication.

Follow-up is critical for reimplanted teeth. We monitor healing at one week, two weeks, four weeks, and three months, looking for signs of ankylosis (fusion to bone), root resorption, or pulp necrosis. Root canal therapy is typically performed 7 to 10 days after reimplantation for mature (closed apex) teeth. Immature teeth in children may retain pulp vitality and are monitored before any intervention. This careful protocol gives your reimplanted tooth the best possible chance of long-term survival.

Understanding Your Investment

At Aloha Dental Specialty Center, we provide transparent pricing before any treatment begins. The cost of your procedure depends on clinical complexity, materials used, and sedation requirements. We walk you through every line item during your consultation so there are no surprises.

What Affects Cost:

  • Clinical Complexity: Bone loss, sinus proximity, nerve involvement, or the need for grafting affects treatment planning and surgical time.
  • Materials: We use research-backed implant systems, purified bone graft matrices, and high-strength ceramics from established manufacturers.
  • Sedation: Options range from local anesthesia to IV sedation, each with different associated costs. We discuss the best option for your comfort and procedure.

Insurance & Financing

We accept most major dental insurance plans and file claims on your behalf. Our team verifies your benefits before treatment and submits pre-treatment estimates with clinical documentation to maximize coverage.

For out-of-pocket costs, we partner with CareCredit and Cherry for flexible payment plans. We also offer our ADSC Dental Savings Plan for patients without insurance.

Recovery & Healing Timeline

Understanding the healing process helps you plan ahead and follow post-operative instructions for the best possible outcome.

First 48 Hours

Mild swelling and tenderness are normal. Apply ice packs in 20-minute intervals and take prescribed or over-the-counter anti-inflammatory medication. Eat soft foods and avoid the surgical site when chewing.

Days 3 to 7

Swelling peaks around day three and then improves. The tissue begins closing over the surgical site. Continue with soft foods and use any prescribed antimicrobial rinse. Most patients return to normal activities during this phase.

Long-Term Healing

Surface tissue heals within two to three weeks. Bone integration and deep healing continue for three to six months. Avoid smoking, follow up as scheduled, and maintain good oral hygiene throughout recovery.

The Science

The Science of Tooth Reimplantation: Time Is the Variable That Matters Most

The International Association of Dental Traumatology (IADT) guidelines, updated in 2020, establish that extra-alveolar dry time is the single most important prognostic factor for reimplanted avulsed teeth. Teeth stored in milk or Hank's Balanced Salt Solution (HBSS) maintain periodontal ligament cell viability for up to six hours, versus 15 to 30 minutes dry. A meta-analysis in Dental Traumatology confirmed that teeth reimplanted within 30 minutes and properly splinted have a five-year survival rate of 85%, compared to less than 10% for teeth reimplanted after two hours of dry storage.

Experience & Expertise

Why Choose a Specialist?

Dr. Jovan Gvozden manages dental avulsion cases using the latest IADT reimplantation protocols. His oral and maxillofacial surgery residency included extensive dento-alveolar trauma training in both the clinic and operating room. Our office maintains flexible wire splinting materials, calcium hydroxide medicaments, and CBCT imaging for immediate post-reimplantation assessment — everything needed to give a knocked-out tooth the best chance of survival.

A Note from Your Dental Team

In my experience, the patients who get the best results from same-day urgent dental evaluation are those who come in with realistic expectations and follow their post-treatment instructions carefully. I’d rather spend extra time explaining what to expect than have a patient be surprised later.

“Every patient I see has a different story, a different set of concerns, and a different definition of what ‘success’ looks like. That’s why I don’t believe in one-size-fits-all treatment plans. When you come in for a consultation, I’ll listen first, examine second, and recommend third.

I also won’t recommend a procedure you don’t need. If your tooth can be saved with a filling instead of a crown, I’ll tell you that. If watchful waiting is appropriate, I’ll explain why. My job isn’t to sell treatment — it’s to give you my honest clinical judgment so you can make an informed decision.

If you have questions about whether this treatment is right for you, or if you’ve been told you need this procedure by another dentist and want a second opinion, call us at (503) 822-0096. We’re happy to take the time to explain everything — no rush, no pressure.”

— Dr. Merat Ostovar & Dr. Jovan Gvozden | Aloha Dental Specialty Center, Beaverton, OR

I know many patients feel anxious about dental procedures — you’re not alone in that. What I hear most often after treatment is: “That was so much easier than I expected.” We’re here to make this as comfortable as possible for you.

Who is a good candidate? Most patients in good general health qualify for same-day urgent dental evaluation. We evaluate your specific situation — including medical history, current medications, and dental condition — during a thorough consultation with CBCT 3D imaging. Factors like uncontrolled diabetes, certain medications, or active infections may require management before proceeding.

Frequently Asked Questions

What should I do immediately if a tooth gets knocked out?

Pick it up by the crown only. Do not touch or scrub the root. If it is clean, try to gently place it back in the socket. If you cannot, place it in cold milk. Call (503) 822-0096 immediately and head to our office. Time is the most critical factor.

Why milk? Can I use water?

Milk has an osmolality and pH similar to living tissue, which preserves the periodontal ligament cells on the root surface. Tap water is hypotonic and causes cell lysis within minutes. Saline or Hank's Balanced Salt Solution are also acceptable, but milk is the most readily available option.

How long do I have to reimplant a knocked-out tooth?

Every minute counts. Reimplantation within 30 minutes gives the best results — up to 90% survival. In milk, viable cells can survive up to six hours, but outcomes decline progressively. Get to our office as fast as safely possible.

Can a baby tooth be reimplanted?

No. Reimplanting a primary (baby) tooth risks damaging the developing permanent tooth underneath. If your child knocks out a baby tooth, control the bleeding with gauze, and call us for evaluation — the permanent tooth space may need monitoring.

What does the splint look like and how long does it stay on?

We use a thin, flexible wire bonded to the reimplanted tooth and its neighbors with composite resin. It looks similar to an orthodontic wire. The splint stays in place for 7 to 14 days, allowing the periodontal ligament to reattach while permitting the small amount of physiologic movement teeth need.

Will I need a root canal after reimplantation?

For mature teeth (closed root apex), root canal therapy is started 7 to 10 days after reimplantation, before the pulp becomes infected. For immature teeth in children with open apices, we monitor for revascularization — the pulp may heal on its own.

What is ankylosis and can it happen to my reimplanted tooth?

Ankylosis occurs when the tooth fuses directly to bone without a normal periodontal ligament. It produces a metallic "high-pitched" sound when tapped and the tooth cannot be moved orthodontically. It is more common with longer extra-oral dry times and is one reason we emphasize speed.

My child knocked out a tooth at a sports game — what now?

If it is a permanent tooth, place it in milk immediately and call (503) 822-0096 while heading to our office. If it is a baby tooth, do not reimplant it — control bleeding with gauze and call us for evaluation. We see pediatric trauma patients the same day.

Can you reimplant a tooth if it has been out for several hours?

We can attempt reimplantation even with extended out-of-mouth time, but the prognosis is significantly reduced. The tooth may survive for years even if eventual ankylosis or resorption occurs, preserving bone and aesthetics until a permanent replacement is planned.

What sports carry the highest risk of tooth avulsion?

Basketball, hockey, soccer, lacrosse, martial arts, football, and cycling account for the majority of tooth avulsion injuries we treat. A custom-fitted sports mouthguard reduces avulsion risk by up to 60 times — we fabricate them in our office.

How much does emergency reimplantation cost?

The reimplantation procedure including splinting and imaging typically ranges from $500 to $1,000. Root canal therapy performed at follow-up is a separate cost. Most dental insurance covers a significant portion of trauma-related treatment.

What if the tooth cannot be saved?

If reimplantation is not possible or the tooth fails, a dental implant is the gold standard replacement. We can preserve the socket with bone grafting and place an implant once healing is complete — typically three to four months. A temporary tooth can be worn in the interim.

Should I go to the emergency room instead?

No. Hospital ERs rarely have the instruments or expertise to reimplant an avulsed tooth. Come directly to us at Aloha Dental Specialty Center — we have the splinting materials, CBCT imaging, and surgical expertise to give your tooth the best chance. Call (503) 822-0096 en route.

What happens during follow-up appointments?

We see you at one week (splint check), two weeks (splint removal), one month, and three months. At each visit, we test vitality, take imaging, and assess for resorption or ankylosis. This monitoring schedule follows the International Association of Dental Traumatology protocol.

Can a reimplanted tooth last a lifetime?

Yes, when reimplanted quickly and managed properly. Teeth reimplanted within 30 minutes with intact periodontal ligament cells have documented long-term survival exceeding 20 years. The key factors are speed, proper storage medium, and diligent follow-up care.
