What a Panoramic X-Ray Shows and When You Need One

Panoramic X-ray Dental is a specialized dental service provided by the board-certified specialists at Aloha Dental Specialty Center in Beaverton, OR. A single image of both jaws — the screening workhorse of dental diagnosis.

Panoramic Radiography

What a Panoramic X-Ray Shows and When You Need One

A panoramic radiograph — also called an orthopantomogram or OPG — captures the entire upper and lower jaw, all teeth, the TMJ condyles, the maxillary sinuses, and portions of the nasal cavity in one continuous image. The machine rotates around your head while you stand still, producing a flattened view of curved anatomy in about 15 seconds. It is the fastest way to get a comprehensive overview of dental and skeletal structures without taking multiple individual films.

At Aloha Dental Specialty Center, a panoramic X-ray is frequently the first imaging obtained for new patients, emergency evaluations, and screening assessments. It reveals impacted teeth, gross caries, bone loss patterns, cysts, fractures, and developmental anomalies at a glance. For straightforward diagnostic questions, it may be all the imaging needed. For complex cases — implant planning, root canal evaluation, surgical extraction assessment — the panoramic film serves as a roadmap that guides the decision to acquire additional focused imaging such as periapical films or a CBCT scan.

The radiation dose from a digital panoramic X-ray is roughly 10 to 25 microsieverts — less than a single day of natural background radiation and significantly less than a CBCT scan. The image is displayed on screen instantly, with the ability to adjust brightness, contrast, and magnification digitally. Dr. Ostovar and Dr. Gvozden review the panoramic film with each patient, pointing out findings and explaining what further evaluation, if any, is recommended.

Understanding Your Investment

At Aloha Dental Specialty Center, we provide transparent pricing before any treatment begins. The cost of your procedure depends on clinical complexity, materials used, and sedation requirements. We walk you through every line item during your consultation so there are no surprises.

What Affects Cost:

  • Clinical Complexity: Bone loss, sinus proximity, nerve involvement, or the need for grafting affects treatment planning and surgical time.
  • Materials: We use research-backed implant systems, purified bone graft matrices, and high-strength ceramics from established manufacturers.
  • Sedation: Options range from local anesthesia to IV sedation, each with different associated costs. We discuss the best option for your comfort and procedure.

Insurance & Financing

We accept most major dental insurance plans and file claims on your behalf. Our team verifies your benefits before treatment and submits pre-treatment estimates with clinical documentation to maximize coverage.

For out-of-pocket costs, we partner with CareCredit and Cherry for flexible payment plans. We also offer our ADSC Dental Savings Plan for patients without insurance.

The panoramic x-ray dental procedure typically takes 30-60 minutes per session. If multiple visits are needed, we’ll space them 2-4 weeks apart. Most patients return to normal activities within 1-3 days.

Recovery & Healing Timeline

Understanding the healing process helps you plan ahead and follow post-operative instructions for the best possible outcome.

First 48 Hours

Mild swelling and tenderness are normal. Apply ice packs in 20-minute intervals and take prescribed or over-the-counter anti-inflammatory medication. Eat soft foods and avoid the surgical site when chewing.

Days 3 to 7

Swelling peaks around day three and then improves. The tissue begins closing over the surgical site. Continue with soft foods and use any prescribed antimicrobial rinse. Most patients return to normal activities during this phase.

Long-Term Healing

Surface tissue heals within two to three weeks. Bone integration and deep healing continue for three to six months. Avoid smoking, follow up as scheduled, and maintain good oral hygiene throughout recovery.

Questions during recovery? Call us at (503) 822-0096. We are available for post-operative concerns.

The Science

How Panoramic Imaging Works

A panoramic X-ray unit uses a narrow, slit-shaped beam that rotates around the patient on a defined path called a focal trough. Only structures within this trough — a curved zone roughly matching the shape of the dental arches — are captured in sharp focus. Structures outside the trough appear blurred or distorted. This is why panoramic images are excellent for screening but limited for precise measurement: magnification varies across the image, and buccal-lingual relationships are compressed into a single plane. Digital panoramic sensors have replaced film in modern practices, providing instant image availability, dose reduction through higher detector efficiency, and post-processing capabilities including annotation, measurement, and integration with electronic health records.

Experience & Expertise

Why Choose a Specialist?

Panoramic imaging at ADSC is performed on a digital unit calibrated for consistent image quality across patient sizes, from pediatric through adult. Every panoramic film is reviewed by the treating specialist — not a technician — in the context of the patient's clinical presentation. When the panoramic image raises a question that requires higher-resolution or three-dimensional evaluation, the CBCT scanner is available in the same suite for immediate follow-up imaging.

Treatment time varies by complexity. A straightforward procedure may take 30-60 minutes, while more involved cases can take 1-2 hours. Your consultation visit typically lasts 45-60 minutes including imaging and treatment planning. We’ll give you a specific time estimate for your individual case.

A Note from Your Dental Team

In my experience, the patients who get the best results from panoramic x-ray dental are those who come in with realistic expectations and follow their post-treatment instructions carefully. I’d rather spend extra time explaining what to expect than have a patient be surprised later.

“Every patient I see has a different story, a different set of concerns, and a different definition of what ‘success’ looks like. That’s why I don’t believe in one-size-fits-all treatment plans. When you come in for a consultation, I’ll listen first, examine second, and recommend third.

I also won’t recommend a procedure you don’t need. If your tooth can be saved with a filling instead of a crown, I’ll tell you that. If watchful waiting is appropriate, I’ll explain why. My job isn’t to sell treatment — it’s to give you my honest clinical judgment so you can make an informed decision.

If you have questions about whether this treatment is right for you, or if you’ve been told you need this procedure by another dentist and want a second opinion, call us at (503) 822-0096. We’re happy to take the time to explain everything — no rush, no pressure.”

— Dr. Merat Ostovar & Dr. Jovan Gvozden | Aloha Dental Specialty Center, Beaverton, OR

I know many patients feel anxious about dental procedures — you’re not alone in that. What I hear most often after treatment is: “That was so much easier than I expected.” We’re here to make this as comfortable as possible for you.

Who is a good candidate? Most patients in good general health qualify for panoramic x-ray dental. We evaluate your specific situation — including medical history, current medications, and dental condition — during a thorough consultation with CBCT 3D imaging. Factors like uncontrolled diabetes, certain medications, or active infections may require management before proceeding.

How long does it take? Most patients complete this procedure in approximately 45-90 minutes. Your initial consultation is approximately 60 minutes including CBCT 3D imaging and treatment planning.

Frequently Asked Questions

What does a panoramic X-ray show?

A panoramic X-ray shows all teeth in both jaws, the roots and surrounding bone, the TMJ condyles, the maxillary sinuses, the nasal septum, and portions of the hyoid bone. It can reveal impacted teeth, cavities, bone loss, cysts, tumors, fractures, and developmental dental anomalies.

Is a panoramic X-ray safe?

Yes. A digital panoramic X-ray delivers approximately 10 to 25 microsieverts of radiation — less than one day of natural background exposure. Lead aprons and thyroid collars are used as additional protective measures. The dose is considered negligible by current radiation safety standards.

How often should I have a panoramic X-ray?

For adults at normal risk, a panoramic radiograph every three to five years is typical. Patients with active dental disease, developing dentitions (children and teenagers), or ongoing treatment may need more frequent imaging. Your doctor will recommend a schedule based on your clinical needs.

What is the difference between a panoramic X-ray and a full mouth series?

A panoramic X-ray is a single image of both jaws taken from outside the mouth. A full mouth series (FMX) consists of 14 to 20 individual intraoral films showing teeth and bone in detail. The FMX provides higher resolution for detecting interproximal cavities, periapical pathology, and fine bone detail. The two imaging types are complementary, not interchangeable.

Do I need to remove jewelry before a panoramic X-ray?

Yes. Remove earrings, necklaces, eyeglasses, hairpins, and any removable dental appliances. Metal objects create bright artifacts on the image that can obscure diagnostic detail.

Can a panoramic X-ray detect oral cancer?

A panoramic X-ray can reveal bone destruction, radiolucent or radiopaque masses, and pathologic fractures that may be associated with malignancy in the jaws. However, soft-tissue cancers of the tongue, cheeks, and throat are not visible on panoramic imaging. A clinical oral cancer screening examination is needed for soft-tissue evaluation.

Why does the panoramic image look distorted?

Panoramic imaging flattens curved three-dimensional anatomy into a two-dimensional image. Objects outside the focal trough appear magnified, reduced, or blurred. The anterior teeth often appear widened. These distortions are inherent to the technique and are accounted for during interpretation. When precise measurements are needed, CBCT or intraoral radiographs are used instead.

Can children have panoramic X-rays?

Yes. Panoramic X-rays are commonly used in pediatric dentistry to evaluate tooth development, detect impacted or missing teeth, screen for pathology, and plan orthodontic treatment. The machine accommodates smaller patients, and the low radiation dose is appropriate for children.

Is a panoramic X-ray uncomfortable?

No. You stand with your chin on a rest and bite gently on a positioning peg while the machine rotates around your head for about 15 seconds. There is no discomfort, no enclosed space, and no physical contact beyond the chin rest.

Can a panoramic X-ray show wisdom teeth?

Yes. Panoramic imaging is the standard screening tool for third molars. It shows whether wisdom teeth are present, their angulation, their proximity to the inferior alveolar nerve, and whether they are impacted against adjacent teeth or bone. This information helps determine whether extraction is recommended.

Does insurance cover panoramic X-rays?

Most dental insurance plans cover panoramic radiographs at 80 to 100 percent, typically once every three to five years. The ADSC billing team verifies your benefits before your appointment and informs you of any out-of-pocket cost.

Why might I need both a panoramic X-ray and a CBCT scan?

The panoramic provides a broad screening overview. If it reveals a finding that requires three-dimensional evaluation — an impacted tooth near the nerve, a lesion of uncertain nature, or a site being planned for implant placement — CBCT provides the detailed volumetric data needed for treatment planning. The panoramic film helps determine whether CBCT is indicated.

Can a panoramic X-ray detect sinus problems?

The maxillary sinuses are partially visible on panoramic imaging. Mucosal thickening, fluid levels, and large sinus lesions can be detected. However, detailed sinus evaluation requires CBCT or medical CT imaging. Incidental sinus findings on panoramic films are communicated to the patient and, when warranted, referred for further evaluation.

How quickly are panoramic X-ray results available?

The digital image appears on screen immediately — within seconds of the exposure. The doctor reviews it with you during the same appointment. There is no waiting for film processing.

Can a panoramic X-ray show TMJ problems?

Panoramic imaging shows the gross morphology of the TMJ condyles and can reveal significant degenerative changes, fractures, or asymmetry. However, the condyle images are distorted by the panoramic projection. When TMJ evaluation is the primary concern, CBCT provides undistorted three-dimensional views of both joints.
