Dental Implants & All-on-X Full-Arch Restoration

Both All-on-4 and All-on-6 are advanced full-arch implant techniques that replace an entire upper or lower arch of missing or failing teeth with a non-removable, permanent prosthetic bridge. The primary difference lies in biomechanical load distribution and bone density.

All-on-4: Employs four implants, strategically tilting the two posterior implants up to 45 degrees to engage dense cortical bone in the anterior jaw. It is ideal for patients with moderate bone loss who want to avoid invasive sinus lifts or bone grafting.

All-on-6: Adds two additional posterior implants for a total of six anchorage points. This creates superior load distribution and surface area integration, which is strongly recommended for patients with softer maxillary (upper jaw) bone, broad dental arches, heavy masticatory bite forces, or those receiving high-density monolithic Prettau zirconia bridges. Learn more on our All-on-4 Guide and All-on-6 Comparison.
Yes. Through our "Immediate Load" All-on-X protocol, any remaining diseased teeth are gently extracted, implants are surgically placed using 3D CBCT guided surgical templates, and a customized, high-strength temporary fixed bridge is securely attached—all in a single clinical appointment. You arrive with damaged or missing teeth and leave our Beaverton office the same afternoon with an aesthetic, functional, non-removable smile. After 3 to 6 months of osseointegration (bone fusion), your final custom monolithic zirconia bridge is seated.
Dental implants have a clinically documented success rate exceeding 95% to 98% over 20+ years. Because titanium implants are biologically inert and fuse directly with living alveolar bone through osseointegration, they cannot develop dental cavities. With proper daily brushing, flossing, water flossing around prosthetic pontics, and bi-annual professional hygiene visits with our periodontist Dr. Walied Eldomiaty, dental implants are designed to serve as a lifelong replacement for natural teeth.
Yes, in the vast majority of cases. Chronic tooth loss or wearing traditional dentures causes the jawbone to resorb over time. However, our specialists utilize advanced bone regeneration techniques, including socket preservation, guided bone regeneration (GBR) with platelet-rich fibrin (PRF), and lateral window or crestal sinus floor elevations. Furthermore, angled implant placement in All-on-4 protocols specifically bypasses resorbed posterior zones by utilizing dense anterior bone structures. We evaluate your 3D bone volume on-site with our high-resolution CBCT scanner.
We prioritize monolithic Prettau zirconia full-arch prosthetics milled from solid blocks of medical-grade biocompatible zirconium dioxide. Unlike older hybrid acrylic dentures with plastic denture teeth that frequently chip, stain, absorb oral odors, and fracture under chewing forces, monolithic zirconia is completely non-porous, highly resistant to chipping, and boasts a flexural strength exceeding 1,200 MPa. It delivers unmatched lifelike translucency and long-term durability.

Periodontics, Gum Disease & LANAP Laser Surgery

Traditional gum surgery (osseous surgery) involves cutting the gum tissue with a scalpel, peeling it back from the bone, scraping away diseased tissue, reshaping bone with burs, and placing sutures. This often leads to significant gum recession, root sensitivity, postoperative pain, and an altered cosmetic appearance.

In contrast, LANAP (Laser Assisted New Attachment Procedure) uses the PerioLase MVP-7 pulsed Nd:YAG laser. The laser light specifically targets and destroys dark-pigmented periodontopathogens and diseased pocket epithelium without cutting or touching healthy tissue. The procedure stimulates mesenchymal stem cells in the periodontal ligament, initiating true regeneration of new bone, cementum, and connective tissue fibers. Patients experience minimal discomfort, no stitches, and can typically return to work the very next day. Learn more on our LANAP Laser Surgery Page.
Key warning signs include: bleeding gums during brushing or flossing; persistent bad breath (halitosis) that does not resolve with mouthwash; red, swollen, or tender gingiva; receding gums making teeth appear abnormally long; periodontal pocket depths measuring 4mm, 5mm, or deeper; pus oozing between teeth and gums; and mobile or shifting teeth. Because periodontal disease is typically painless in its early and moderate stages, many patients do not realize severe bone destruction has occurred until teeth become loose.
Yes. Peri-implantitis—an inflammatory condition causing bacterial bone loss around existing dental implants—can be effectively treated using LAPIP (Laser Assisted Peri-Implantitis Procedure). Dr. Walied Eldomiaty utilizes the PerioLase MVP-7 laser to vaporize bacteria and sterilize the titanium implant threads without damaging the implant surface or generating excessive thermal heat that harms surrounding bone. This stops progressive bone loss and promotes re-osseointegration.
Dr. Eldomiaty offers multiple advanced soft tissue grafting techniques: (1) Connective tissue autografts; (2) Minimally invasive Pinhole Surgical Technique (PST) and tunneling protocols that reposition existing gums through micro-punctures without large incisions; and (3) Acellular dermal matrix (allografts) combined with Platelet-Rich Fibrin (PRF) derived from your own blood to accelerate biological healing, cover exposed sensitive roots, and thicken delicate gingival biotypes.

Endodontics, Root Canals & Tooth Preservation

The persistent myth that root canals are painful stems from outdated techniques used decades ago. In reality, a modern root canal does not cause pain—it relieves severe pain caused by infected or necrotic nerve tissue.

Under the care of our specialized endodontic team, we utilize profound local anesthetics and advanced buffering techniques so that the tooth is completely numb before treatment begins. Most patients report that having a root canal under our surgical operating microscope feels no different than getting a standard dental filling. For anxious patients, nitrous oxide or oral conscious sedation is readily available. Learn more on our Root Canal Specialist Page.
Occasionally, a tooth that received a root canal months or years ago fails to heal or develops new infection due to microscopic narrow canals that were unaddressed, delayed placement of the final crown, salivary contamination, or a new fracture. During retreatment, our endodontic specialists remove the previous gutta-percha filling material under high-power surgical magnification, clean and sterilize complex micro-canals with ultrasonic irrigation, disinfect with antibacterial intracanal medicaments, and reseal the roots using mineral trioxide aggregate (MTA) or bioceramics to save the tooth from extraction.
An apicoectomy is a microsurgical procedure performed when infection persists in the bony area around the apex (tip) of a tooth root despite root canal therapy. Rather than extracting the tooth, our endodontic specialists create a small microscopic opening in the gum tissue near the root tip, remove the infected tissue and the last 3mm of the apex, and seal the root canal from the bottom with a biocompatible bioceramic retrograde filling, preserving the functional natural crown.
Nothing looks, feels, or functions exactly like your natural biological tooth. Natural teeth possess a delicate periodontal ligament (PDL) equipped with mechanoreceptors that provide sensory feedback, allowing you to gauge bite pressure precisely. Preserving the tooth maintains natural bone architecture and avoids the need for neighboring tooth preparation or complex surgeries. When a tooth has a restorable crown and sound periodontal foundation, endodontic preservation is almost always the most cost-effective and biologically conservative choice.

Pediatric Dentistry & Infant Oral Health

A tongue-tie (ankyloglossia) or lip-tie occurs when the thin band of tissue beneath the tongue or upper lip is abnormally short, thick, or tight, restricting normal oral mobility. In infants, this causes painful shallow latches during breastfeeding, poor weight gain, excessive gas, and reflux. In toddlers and older children, it can contribute to speech delays, dental spacing, and airway restrictions.

Our pediatric team performs gentle infant frenectomies utilizing a specialized dental soft-tissue laser. The laser precisely releases the restrictive tissue in under 90 seconds with virtually no bleeding, no stitches, and instant pain relief. Nursing mothers can nurse immediately following the procedure in our comfortable private consultation room. Explore our Pediatric Dental Specialist Page.
Yes! For early or moderate childhood dental caries, our pediatric team utilizes Silver Diamine Fluoride (SDF)—an FDA-cleared, non-invasive liquid antimicrobial treatment. Painted directly onto the decayed enamel with a tiny brush in seconds, the silver ions kill cariogenic bacteria while fluoride remineralizes the tooth structure, halting cavity progression completely without drills, local anesthetic injections, or tears.
Primary ("baby") teeth serve vital functions: they allow children to chew nutritious solid foods comfortably, assist in clear speech development, and serve as natural space maintainers for developing adult permanent teeth. If primary teeth are lost prematurely to decay or infection, adjacent teeth shift into the vacant space, causing severe crowding and impaction of permanent teeth. Untreated dental infections in baby teeth can also spread systemically or damage the developing permanent tooth buds directly beneath them.

Sedation Dentistry & Anxiety-Free Care

Moderate intravenous (IV) sedation is often called "twilight sleep." Unlike general anesthesia performed in a hospital setting with endotracheal breathing tubes, you remain safely breathing on your own. Medications administered directly into the bloodstream induce an immediate state of deep physical and psychological relaxation. You will be free from pain and anxiety, and time will seem to pass in just moments. Most patients have zero memory of the surgery, drills, sounds, or smells. Learn more on our IV Sedation Dentist Page.
Patient safety is our foremost priority. Our clinical sedation suites are equipped with hospital-grade multi-parameter patient telemetry monitors. Throughout your procedure, our trained surgical staff continuously monitors: (1) End-tidal CO2 capnography (measuring real-time ventilation and breath rate); (2) Pulse oximetry (SpO2 oxygen saturation); (3) Continuous 3-lead electrocardiogram (ECG heart rhythm); (4) Automated non-invasive blood pressure (NIBP); and (5) Core body temperature. Reversal agents and full emergency airway equipment are present in the surgical suite at all times.
Key pre-operative requirements include: (1) Fasting: Do not eat or drink anything (including water, coffee, or juice) for at least 6 to 8 hours prior to your scheduled surgery time; (2) Responsible Adult Escort: A responsible adult must drive you to your appointment, remain in our clinic during the procedure, and drive you home; (3) Clothing: Wear comfortable, loose-fitting clothing with short sleeves to facilitate blood pressure cuffs and IV access; and (4) Medications: Take approved daily medications with a tiny sip of water as specifically instructed during your pre-sedation medical evaluation.

Advanced Dental Technology & 3D Imaging

Cone Beam Computed Tomography (CBCT) produces volumetric 3D cross-sectional images of the teeth, jawbones, nerve pathways, and sinus cavities in a single 14-second scan. Unlike medical hospital CT scanners that expose patients to high radiation doses, dental CBCT uses a cone-shaped X-ray beam that emits up to 90% less radiation. It provides sub-millimeter diagnostic precision for computer-guided implant placement, complex root canal anatomy detection, and airway evaluation. Learn more on our 3D CBCT Imaging Page.
