Anxiety Management / Comfort Dentistry
Anxiety Management Comfort Dentistry is a specialized dental service provided by the board-certified specialists at Aloha Dental Specialty Center in Beaverton, OR. You are not weak for being afraid of the dentist — you are normal, and we know how to help.
Dental Fear Is Real, Common, and Treatable — Here Is How We Approach It
Roughly 36 percent of the population has some level of dental anxiety, and between 12 and 20 percent have anxiety severe enough to avoid dental care entirely. If you are reading this page, you probably fall somewhere in that range. Years of avoided treatment create a cycle: the longer you wait, the more work you need, and the more overwhelming each visit feels. At Aloha Dental Specialty Center, we break that cycle by meeting you exactly where you are — no judgment, no rushing, and a genuine understanding that dental fear is a physiological response, not a character flaw.
Our approach to anxiety management is layered and customized. For mild anxiety, a calm explanation of what is happening, noise-canceling headphones, and a sense of control (raise your hand and we stop immediately) may be all you need. For moderate anxiety, nitrous oxide — inhaled through a comfortable nasal mask — takes the edge off within minutes and wears off equally fast. For severe anxiety or dental phobia that has kept you out of a dental office for years, IV sedation provides deep relaxation with little to no memory of the procedure afterward. Dr. Ostovar and the clinical team assess your anxiety level at the consultation and recommend the appropriate approach before any treatment begins.
What most anxious patients do not realize is that modern dental treatment at a well-equipped specialty office is nothing like the experience that created their fear. The drills are quieter. The anesthetics are more effective and faster-acting. The imaging is digital and instantaneous. The communication is transparent — you see what we see on the monitor, and every step is explained before it happens. Many of our most anxious patients tell us after their first visit that they wish they had come in years ago. If dental fear is the only thing standing between you and the care you need, call (503) 822-0096. We will take it at your pace. Office hours: Monday through Friday 7 AM to 7 PM, Saturday and Sunday 8 AM to 2 PM.

Understanding Your Investment
At Aloha Dental Specialty Center, we provide transparent pricing before any treatment begins. The cost of your procedure depends on clinical complexity, materials used, and sedation requirements. We walk you through every line item during your consultation so there are no surprises.
What Affects Cost:
- Clinical Complexity: Bone loss, sinus proximity, nerve involvement, or the need for grafting affects treatment planning and surgical time.
- Materials: We use research-backed implant systems, purified bone graft matrices, and high-strength ceramics from established manufacturers.
- Sedation: Options range from local anesthesia to IV sedation, each with different associated costs. We discuss the best option for your comfort and procedure.
Insurance & Financing
We accept most major dental insurance plans and file claims on your behalf. Our team verifies your benefits before treatment and submits pre-treatment estimates with clinical documentation to maximize coverage.
For out-of-pocket costs, we partner with CareCredit and Cherry for flexible payment plans. We also offer our ADSC Dental Savings Plan for patients without insurance.
Recovery & Healing Timeline
Understanding the healing process helps you plan ahead and follow post-operative instructions for the best possible outcome.
First 48 Hours
Mild swelling and tenderness are normal. Apply ice packs in 20-minute intervals and take prescribed or over-the-counter anti-inflammatory medication. Eat soft foods and avoid the surgical site when chewing.
Days 3 to 7
Swelling peaks around day three and then improves. The tissue begins closing over the surgical site. Continue with soft foods and use any prescribed antimicrobial rinse. Most patients return to normal activities during this phase.
Long-Term Healing
Surface tissue heals within two to three weeks. Bone integration and deep healing continue for three to six months. Avoid smoking, follow up as scheduled, and maintain good oral hygiene throughout recovery.

The Psychology and Neuroscience of Dental Anxiety
Dental anxiety is classified in clinical psychology as a specific phobia (dentophobia or odontophobia) when it leads to persistent avoidance of dental care. Neuroimaging studies show that dental-anxious patients exhibit heightened amygdala activation — the brain's fear-processing center — in response to dental stimuli, even when no pain is actually present. The anxiety is not imagined; it is a measurable neurological event. Cognitive-behavioral research demonstrates that graduated exposure, perceived control, and predictable communication significantly reduce physiological stress markers (cortisol, heart rate) in dental patients. Pharmacological interventions — nitrous oxide, oral benzodiazepines, IV sedation — provide a complementary biological layer by modulating GABA-A receptor activity in the central nervous system, reducing the excitatory neural signaling that drives the fear response. The most effective anxiety management combines behavioral and pharmacological strategies tailored to the individual patient.
Why Choose a Specialist?
Aloha Dental Specialty Center has treated thousands of patients with dental anxiety ranging from mild apprehension to severe phobia that caused years of dental avoidance. Dr. Ostovar and the clinical team use a structured anxiety assessment at the initial consultation to determine the appropriate level of support — behavioral, pharmacological, or both. IV sedation is available on-site for patients who need it, administered with continuous monitoring and reversal agents immediately accessible. The office environment is designed with anxious patients in mind: private operatories, a calm clinical atmosphere, and a team that never rushes or dismisses a patient's fear.

A Note from Your Dental Team
In my experience, the patients who get the best results from anxiety management comfort dentistry are those who come in with realistic expectations and follow their post-treatment instructions carefully. I’d rather spend extra time explaining what to expect than have a patient be surprised later.
“Every patient I see has a different story, a different set of concerns, and a different definition of what ‘success’ looks like. That’s why I don’t believe in one-size-fits-all treatment plans. When you come in for a consultation, I’ll listen first, examine second, and recommend third.
I also won’t recommend a procedure you don’t need. If your tooth can be saved with a filling instead of a crown, I’ll tell you that. If watchful waiting is appropriate, I’ll explain why. My job isn’t to sell treatment — it’s to give you my honest clinical judgment so you can make an informed decision.
If you have questions about whether this treatment is right for you, or if you’ve been told you need this procedure by another dentist and want a second opinion, call us at (503) 822-0096. We’re happy to take the time to explain everything — no rush, no pressure.”
— Dr. Merat Ostovar & Dr. Jovan Gvozden | Aloha Dental Specialty Center, Beaverton, OR
I know many patients feel anxious about dental procedures — you’re not alone in that. What I hear most often after treatment is: “That was so much easier than I expected.” We’re here to make this as comfortable as possible for you.
Are You a Candidate for Anxiety Management Comfort Dentistry?
As with any dental procedure, there are potential risks including infection, temporary discomfort, and in rare cases, complications that require additional treatment. We minimize these risks through 3D imaging, sterile protocols, and careful case selection. During your consultation, we’ll discuss any specific risks related to your individual situation.
Most patients in good general health are candidates for anxiety management comfort dentistry. However, certain factors may affect your eligibility or require modifications to the treatment plan:
- Medical conditions: Uncontrolled diabetes, autoimmune disorders, or blood-thinning medications may require coordination with your physician before treatment.
- Bone quality: For anxiety management comfort dentistry involving the jawbone, adequate bone density is essential. A CBCT 3D scan during your consultation determines this precisely.
- Smoking: Tobacco use significantly impairs healing. We strongly recommend quitting 2-4 weeks before and after any surgical procedure.
- Age: There is generally no upper age limit. What matters is your overall health, not your age. We have successfully treated patients in their 80s and 90s.
The only way to know for certain is a consultation with our board-certified specialists. Call (503) 822-0096 to schedule your evaluation — we’ll tell you honestly whether this is the right treatment for your situation.
Typical Healing Timeline:
Days 1-3: Initial healing, mild swelling managed with ice and medication. Days 4-7: Swelling subsides, gradual return to normal diet. Weeks 2-4: Soft tissue heals completely. Months 2-6: Bone remodeling and full integration (for surgical procedures). Individual recovery varies — we provide detailed post-op instructions specific to your procedure.

Frequently Asked Questions
Is it normal to be afraid of the dentist?
Extremely normal. Studies estimate that 36 percent of the population experiences dental anxiety, and 12 to 20 percent have anxiety severe enough to delay or avoid treatment entirely. You are not alone, and your fear does not make you a difficult patient — it makes you a patient who needs a different approach.
I have not been to the dentist in years because of anxiety. Will you judge me?
No. We see patients who have avoided dental care for five, ten, even twenty years. Our only concern is helping you get healthy from this point forward. We assess what needs to be done, prioritize by urgency, and work at a pace you are comfortable with. No lectures, no judgment.
What sedation options do you offer for anxious patients?
We offer a range: nitrous oxide (laughing gas) for mild to moderate anxiety, oral sedation with a prescribed medication taken before your appointment, and IV sedation for severe anxiety or lengthy procedures. The right option depends on your anxiety level, the procedure, and your medical history.
What is nitrous oxide and how does it work?
Nitrous oxide is a mild sedative gas inhaled through a small nasal mask. It takes effect within two to three minutes, producing a feeling of relaxation and mild euphoria. You remain fully conscious and can communicate normally. When the gas is turned off, the effects disappear within five minutes. You can drive yourself home afterward.
Can I listen to music or use headphones during treatment?
Absolutely. Many patients bring their own headphones and listen to music, podcasts, or audiobooks during treatment. Reducing auditory input from dental instruments is a simple but effective anxiety-reduction strategy. We will communicate with hand signals or pauses in the music when needed.
What if I need to stop during a procedure because I am feeling overwhelmed?
Raise your hand and we stop. Immediately. No questions, no frustration. We build breaks into the treatment plan for anxious patients, and the ability to pause and regroup at any time gives you a sense of control that is one of the most effective anxiety-reduction tools we have.
How do you numb the tooth without causing pain?
We apply topical anesthetic gel to the gum tissue for two to three minutes before any injection. The initial needle placement uses the thinnest gauge needle available and a slow injection technique that avoids the sharp sting of rapid fluid delivery. For patients on IV sedation, the local anesthetic is given after sedation — you will not feel or remember the injection.
I gag easily and that makes dental visits terrible. Can you help?
Yes. A strong gag reflex is a common contributor to dental anxiety. Nitrous oxide reduces the gag reflex significantly, and IV sedation virtually eliminates it. For impressions, digital intraoral scanning is far more comfortable than traditional impression trays. We have multiple strategies to manage this.
Can all procedures be done under sedation?
Yes. Extractions, root canals, implant surgery, bone grafting, and even diagnostic evaluations can be performed under sedation if your anxiety warrants it. The sedation approach is selected based on the procedure length, complexity, and your individual anxiety level.
Will sedation eliminate all pain during the procedure?
Sedation eliminates anxiety and awareness. Local anesthesia eliminates pain. Both are used together. The sedation ensures you are calm and comfortable, and the local anesthetic blocks pain signals from the treatment area. Together, they create a pain-free, anxiety-free experience.
How much does sedation add to the cost of treatment?
Nitrous oxide adds a modest fee per visit. IV sedation is a larger additional cost that depends on the duration of the procedure. We provide exact pricing as part of your treatment plan. For many patients, the cost of sedation is justified by the ability to receive treatment they would otherwise avoid entirely.
Can I have someone come with me for moral support?
Yes. You are welcome to bring a family member or friend to the consultation and even into the operatory if space permits. For IV sedation appointments, a companion is required to drive you home. Having a familiar person nearby can significantly reduce anxiety, especially at the first visit.
What if my anxiety is so bad I cannot even make the phone call?
You can book online through our website without speaking to anyone. You can also have a family member or friend call on your behalf. Our scheduling team understands dental anxiety and will be patient and reassuring during the call. The first step is the hardest — everything gets easier from there.
Will I need sedation every time I come in?
Not necessarily. Many patients who start with IV sedation find that their anxiety decreases after a positive experience. Some transition to nitrous oxide for routine procedures and reserve IV sedation for more involved work. We reassess at each visit and adjust the approach based on how you are feeling.
My previous bad experience was a dentist who would not stop when I asked. How is ADSC different?
That experience is one of the most common origins of severe dental anxiety, and we take it seriously. At ADSC, you have complete control. If you signal to stop, we stop — no exceptions, no finishing the current step, no convincing you to push through. Your consent and comfort are non-negotiable at every moment of every visit.
